Caring for a loved one can be a source of immense strength and love, but it also comes with challenges. If you’re over 18 and support a family member who needs your help due to illness, disability, or age-related needs, Carers Trust Tyne & Wear is here to empower you.

What you can expect from us:

To ensure a perfect fit, a Care Coordinator will visit your home to meet you and your loved one. This helps us understand your needs and tailor our support. We’ll then conduct a Care Needs Assessment to create a personalised plan for when and how our Carer Support Workers can best assist you.

Your care plan

A Care Coordinator will create a personalised care plan outlining the tasks you want the Carer Support Worker to handle, including medication and personal care. This plan is risk-assessed for everyone's safety and used by the Carer Support Worker during each visit to guide and document the provided services.

When your service starts

After finalising the care plan, we'll confirm your start date, your Carer Support Worker, and preferred service times. We prioritise consistency, but if needed, a replacement will be arranged for illness or holiday absence. For any questions or changes, a Care Coordinator is available during office hours (M-F) or 24/7 on-call.

Reviewing your service

Once your service has begun, we’ll be in touch after 6 and 12 weeks to check that everything is running smoothly. Each year, we’ll carry out a review to ensure that the service continues to meet your needs. Carers can contact us at any time if there’s an aspect of your service that you want to discuss.
